Brookwood Family Medicine will be closing this fall as Dr. DeJarnette & Dr. Copas
will be retiring. The office will be open, business as usual, until the expected
closure date set for September 29, 2017.
We are no longer accepting new patients.
No appointments or walk-in visits will be available after September 22, 2017.
Our website will be updated with additional information regarding the closure as it becomes available.
Patients may use the release of information form below to obtain a copy of your medical records.

Please print the release form, fill in the requested areas making sure to clearly designate where
the medical records are being sent, # 2 on the form, sign and date the form then return it to our
office by fax at 770-979-8827 or 770-979-8353 or you may send by mail to:

Note: 1st copy of records will be free of charge, subsequent copies will be subject to allowed
fees per Georgia State Copy Law, Section 2: Code Section: 3-33-3.

Note: Call the office's provided in the list to ascertain if they are accepting new patients and if they
are in network with your insurance plan. If you do not find a provider within this list please speak with
family or friends to see who they recommend, call your insurance provider to get a list of local
in network providers or contact your local hospital for a list of local primary care providers.
Established patient's: Office Visits are by appointment only for routine needs such as refills, lab discussions, or physical exams. Established patients may now choose to utilize our Walk-In hours for Sick / Illness related issues. These hours are Monday-Friday from 9:30-10:30 AM or Monday-Thursday from 2:15-3:30 PM. These visit types are appropriate for Sick/Illnesses related issues limited to 1 complaint only. It is not acceptable for Urgent / Life-threatening issues. Walk-in visits are seen per physician availability and should expect extended wait times, patients are still encouraged to call ahead to avoid lengthy delays. New patients are not acceptable as Walk-In's.
